VA bank not active in Midtrans

Symptoms

• BCA VA does not return a VA number
• Midtrans returns 402 Payment channel is not activated
• Order payment instruction fails for a specific bank

Likely causes

• That bank channel has not been activated for this Midtrans merchant

How to fix

• Use an active VA bank: BNI, BRI, Permata, or CIMB
• Activate BCA VA in Midtrans Dashboard, then add `bca` to `MIDTRANS_ENABLED_VA_BANKS`
